  "text": "Experience does not guarantee learning\n\nOrganisations can experience the same problem repeatedly without changing. People may understand what went wrong but lack authority, time or incentives to alter the system around them.\n\nAdaptation begins with observation, but observation must travel. A lesson needs language that another unit can understand, evidence strong enough to survive challenge and an owner able to act on it.\n\n Local solutions can stay local\n\nImprovisation is valuable because it responds quickly to immediate conditions. It can also remain trapped inside one team. When the people leave, the solution disappears with them.\n\nInstitutional learning requires a mechanism to compare local experience, test whether it applies elsewhere and change training, doctrine or equipment accordingly. That process is slower than improvisation because it must distinguish a durable pattern from a temporary exception.\n\n Adaptation is selective\n\nAn organisation that changes everything after every surprise becomes unstable. Learning also means protecting practices that still work and rejecting lessons built on weak evidence.\n\nThe adaptive institution is therefore not the one that changes fastest. It is the one that can connect evidence to decision, test the result and revise again without losing coherence.",
